PRIVACY POLICY for BODY IQ

Body IQ practice and its practitioners are committed to upholding your privacy and preventing disclosure of your personal and health information to unauthorised third parties.

Kind of information collected and its purpose

  • We collect information only as necessary to enrol you as a new patient, review and update your existing patient details. In health care consultations, we ask questions about your lifestyle, wellbeing, activity, health history and/or disabilities. We ask this so that we can provide tailored health services and referral pathways for you or the person you are the Guardian or substitute decision maker for.


Passing privacy at clinic intake

  • We request points of identification for new patients to prevent fraud, prevent unauthorised use of your and other patients names’. We verify the identity of existing patients by cross checking existing details on record.
     Record storage, management and relay to third parties.

  • We secure your records in an encrypted filing system (JaneApp and Cliniko.).

  • Unless we are subpoenaed or required under law or a legal mandate, we will always seek your consent before relaying your records to a third party. If a subpoena is received, you will be informed however. Please note that compensable injury management schemes, courts, tribunals, insurers and private health insurance funds can request practice records for auditing or case review purposes.

  • Practitioners employed by BodyIQ have approval to access their patients clinical records if directly related to treatment of you (the patient) and your condition.


Referral to other health professionals

  • We seek your consent, either verbally or in writing, before we refer you and send a summary of your presenting condition to any other health professionals for assessment or clinical management that is outside of BodyIQ.


Third parties joining a consultation for you 

  • Unless you give prior consent to a third party person being included and present in your consultation, no person other than you will be present. Please note that compensable injury management schemes like work place injury schemes, motor vehicle accident schemes, courts, tribunals or insurers can request a delegated case manager or party be present in your consultation if they are rebating or subsidising treatments provided to you by us. When a third party with authority seeks to be involved in your consultation or a case conference, you will be informed.

  • If a patient is unable to self-consent, i.e. are a child or have compromised decision making abilities, we will work with Guardians or substitute decision makers to uphold that patient’s privacy. This may require sharing personal/health information with legal Guardians and/or Substitute decision makers.
     Occasions where we may need to disclose your information without your self-consent (serious health episodes on site, disclosed intent to harm self or others.

  • If you have a serious health episode on premises and become unconscious, please be advised that we may have to share aspects of your personal or health information as needed to render appropriate emergency or lifesaving services. 

  • If you disclose intent to harm yourself or others at a particular point in time, we may need to disclose this information as required under the laws of the jurisdiction.
